Patient self check in to include demographic update when completing online intakes

When patients fill out the intake forms, they tend to leave the "financial responsibility" section blank if they are using insurance. Also, when parents fill it out for their children, they put their information in that section, and there's nowhere for them to list the minor's (patient's) information.
