Intake designed to increase completion
We receive so many "completed" intakes that are not in fact the complete picture of their health. We are shocked when someone actually completes one complaint section and we rarely see someone complete more than one even though they come in reporting more than one. The design of the intake does not encourage patients to provide a complete depiction of what is bringing them in.

CallieWe receive intakes also that say "completed" however they have only completed their demographics and only the severity of one complaint with no other information at all. We end up having to get them to complete forms in the office. Perhaps implementing a feature that will not allow them to move on to the next section until they have answered all questions that apply.
