Online Intake- Multiple suggestions

1. On patient intake from please place red asterisk next to "Insurance selection" to show its required.
2. On patient intake, please place button "not applicable" next to Claim Information. Or better yet, a side button that says "I have Personal Injury or Workers Comp Claim Information to report" then if they slide it, that section will show.
3. Under the lifestyle drinking and smoking section, it would make more sense to have something like "Date habit began" as it is a more clear question, or simply not have a "date" question as it is unlikely anyone knows the date they became a light drinker or a light exerciser. Thanks!
