Add pop up for more complaints on Intake forms to ensure patient enters all complaints

Upon finishing adding a complaint, a pop up should be added to ask if they would like to add another complaint. The button at the bottom frequently goes unnoticed or missed and patients tell me them during exam which is a time suck.
