Add Repeatable Outcome Assessment Forms Beyond New Patient Intake

ChiroFusion does a good job with digital intake forms for new patients, but there is still a major gap for repeatable digital clinical forms used during active care.

Our office would like the ability to create and send repeatable patient questionnaires and outcome assessments for established patients, not just intake paperwork.

-Back Bournemouth
-Roland-Morris
-NDI
-ODI
-Other custom symptom/progress tracking forms

Needed workflow:

-Custom fillable digital forms
-Assign to existing patients at any point in care
-Send before appointments or complete in-office on tablet/iPad
-Automatically save to the patient chart
-Auto-score assessments where applicable
-Show score history over time
-Option to save/export completed form as PDF in chart documents

This would significantly reduce paper waste and manual scanning, while improving documentation, patient tracking, re-exams, PI case management, and insurance support.

This would be especially helpful for chiropractic offices using recurring functional/outcome measures throughout a plan of care, not just at intake.
