Box for patients middle initial as an option instead of having to put it with the first name

  A box for patients suffix is need as well. Currently having to add to last name (e.g. Smith Jr or III)

  Please add a field for middle name (not just middle initial) that is optional, not a required field. Many patients go by their middle name.

  Also a field for preferred name. We have so many patients that either go by their middle name or a nick name. Would be nice to have the option to do a patient search using the preferred name.

  More insurances are requiring a middle initial in the actual middle initial box on a HCFA. Kind of crazy that there isn't a way to do that

  Currently, there is no place to put in the client's middle name in their profile optionally, which is a VERY needed for entering clients into the FMCSA. Therefore, we are having to place this information into the "notes" box to ensure that we have it. This option would be an immense help for many of our businesses alike.
