Create Addendum Note
In addition to Exam notes and SOAP notes, an addendum should be created. With the ability to be created as an additional document on the same day of service as a SOAP or EXAM note.

AIt leave a better and more secure paper trail when you have the ability to append an addendum to an existing note. Deleting a note and rewriting is a red flag for insurance companies. Please implement this.

Joshua BorgardtNot being able to amend a document is quite limiting. Please do more than consider; As it will in the end for you, help with your database heterogeneity. I realize you will have to have someone write the database with improved layering, but it is a very important feature for our clinical practices. We are not all perfect, and at times make mistakes that need to be amended.

Paul MahlerIf you just include an addendum section and allow editing of the notes which has a huge voting section above then that would take care of both of these issues at once and would be terrific

Kris Moulton MergedIt'd be helpful to be able to update SOAP notes instead of starting over. Sometimes we need to add something that was missed or relevant to a visit, but we can't do that now. We should be able to make amendments with date, user, and so on, so all changes are documented.
