Have a "Create an Exam/Re-exam" next to "Create a SOAP Note" on pop up screen

Ability to automatically click to create an exam note instead of starting a SOAP note and changing over.

When we go from the schedule to the pop up, the pop up currently has a button that says, "create SOAP note". Please add another button right beside it that says "create EXAM note" so there are fewer clicks to go through to set up the proper screen and date.

On the menu from the daily appointment, there should be a button for "Create an Exam Note" instead of just a daily note.

Have the ability to create a new exam entry from schedule without having to use "create soap note" and then having to change visit type to exam and reenter the DOS.
