Adding - enter manual complaint manually or Stomach - to intake forms
We treat a variety of conditions in our office that are not limited to Chiropractic.
It would be extremely helpful to have an update to the intake form for this purpose.
Adding: Enter manual complaint for the patient to fill out
OR
Add - Stomach to the drop down menu
on the intake forms.

patient that come see us and fill out an intake online cant choose stomach or abdomen. there must be a lot of chiropractors that work with nutrition and digestive issues. [please add that as an option when fill out intake form. -
