Improve Intakes-Eliminate password, add body so patient's can mark

Eliminate having to create a log in & password to fill out intakes. Add an anatomical body to marks areas where there is pain and or a concern.

Yes , we have received complaints from patients about creating a password and some have even decided against filling the intakes out because of this process.
