Paper intakes to match online intakes

Our office would like to be able to remove tabs from the online intake forms (our office does not use Insurance, therefore we do not use the "Insurance" tab). We would like our in-office paperwork to be abe to match the online paperwork as well (AKA; printability). And/or uploadability, being able to scan our in-office paperwork into the online itnake forms would be nice. -

TracieI would like to go a step further and have a link available for forms that patients can easily access or that we could send via email for patient's to click through and fill out to send back. The intake does not seem to work for any of our patients and we would like to have some other format that would be easy for them to fill out online or for us to print out at the office.

ChristopherAs it stands now, this link to fill out paperwork cannot be accomplished on a mobile device, at least cell phones that is.
We basically created our very own paper copies to match the online intake. Even with a tablet or kiosk for patients to do it in office who do not have email or are not comfortable with it, want to fill out a paper copy. -

Annie WMaking the online intake form more user friendly over all would be greatly appreciated. Currently it becomes such a hassle. It is something that is so nice to have for office flow. We often find ourselves having to use a paper version. It would at least be nice to print the online version but please make this feature more user friendly.

KelseyDesperately would like paper forms to match the online intake. We have many patients who are not technologically savvy, or have physical limitations to their ability to complete online. Many of our elderly patients may not comprehend how to use their email to gather the link. Using a table or desktop to fill out these forms is practically impossible without careful and constant assistance.

Justine W.The online intake paper work process is a huge headache for us, especially with older or computer illiterate patients. We would like the ability for the patient's to complete a paper copy if they choose. I have noticed that their biggest struggle to complete the paper work is the process of "registering their account" with the email given and creating a password. I do not feel a password is necessary, since I believe none of our patients are going to log back in to make any changes.

Pamela HillAbility to remove or modify the verbiage on the insurance tab. We are a cash practice with the exception of Medicare. Even though I tell our new patients over the phone that we do not accept insurance, they fill out the insurance form, because it is a mandatory form, and believe that we do take insurance because it is on the form.
