  As healthcare providers we need to provide a safe space for all of our patients. The male/female gender option is outdated and should be changed to be inclusive to all people.

  Add Gender option for those who want to include it in the intake. It is imperative that we know when someone is in transition or has transitioned medically for their treatment. It is also respectful to the client. I understand not all think this is a positive change so making it optional for the practice to add it themselves may help.

  We've had multiple patients complain about being asked for Gender and only having two options on the Intake Form Might make things easier to just ask for Sex instead.

  Rather than asking for genders, ask sex at birth (or both). Sex at birth needs to be required.
