David BeaversThe quadruple VAS form appears to be incorrect. I teach OATS at Logan University and I only see three of the quad on the form, am I missing something here? There are "now", "average" and "worst" on your version; however, you have left off "best" or at least I cannot find it.

KristinCustom questions on check in would help in multiple ways. Chirotouch allows the patient to sign in with a tablet and report what they are coming in for that day. The patient can also report if they are better or worse since last visit and rate their pain. Please consider this ASAP. I have requested this before and was told no.

Jason KASTER MergedI was told there was patient self check in when I purchased this system; with such a thorough system I am not sure why this has not been implemented. I now have to hire someone to go into the rooms and sign in with my credentials to acquire the subjective complaint (which means if a staff member is in another room signed in under my credentials obtaining the subjective complaint I can not pull up the patient I am working on in the other room). My old system acquired the subjective complaint upon check in. At a minimum staff should be able to sign in with their credentials to obtain the subjective complaint.

John HimleyIt would be GREAT to have an option, that I control, to allow patients to enter some level or levels of subjective input and a VAS. Also give me the option of turning off things like the "click a provider'" button on check in ( I'm the only provider here) , turning off the reason for your visit. Its not needed for me. And also turning off "walk in or appt" These things are not needed.... Thanks

As of right now, I have to export a report and use mail merging to print a form for each patient to verify their contact info and to indicate whether their insurance has changed.

michael Panzarino MergedI would like patients to be able to fill out subjective complaints at the front desk Kiosk; where is pain today, scale of 0-10 ect. Having patients fill it out them selves at check in before getting adjusted would save me a ton of time and allow me to track their progress more precisely over time.

Matt W MergedAllowing patients to fill out a subjective history each visit would help streamline notes and be able to track patient progress a lot smoother. This would allow patients to have something to do while they wait to get taken back by the chiropractor and the history to populate in the patient’s SOAP note for the day. This would be very helpful in busy practices that see a high volume of patients.
