Add upper extremity functional index / lower extremity functional index to outcomes assessments.

There are many other assessments that could be added, but I find that these two are used very frequently in our practice. We currently have patients fill them out on paper and scan them into their file, but obviously we are trying to get rid of as much of the paper in our office as possible.
