Eliminate intake form password creation
Can we eliminate the whole creating a password thing to do intakes? Patients get so frustrated with this step.

Not all elderly patients have an email address, let alone a computer. We need options for patients to sign in without an email address -

LoriIntakes need to be super simple, not everyone has email. From my experience, the patient's leave a lot of important info undone and they still can complete the form. Today's patient for me didn't even put in reason for the appt, didn't add her address/contact info and it let her complete the intake, when asked she said she filled out the info, so unsure why it came thru blank on my end. Intakes are very frustrating for the patients and staff as well, I'd rather do a paper application and input everything myself.

Judit ChildI was given a reason by ChiroFusion as to how a patient could 'complete' an intake and it still not show (because the patient didn't enter it) their name, demographics or even Complaint information - however, I don't remember the reason and I'd still be perplexed even if I did. It does not make sense to have an intake which they patient can leave virtually blank. Last intake I received showed 100% completion but had NO name, demographic or insurance info and only of "5 of 10" under the Complaints. PLEASE make it so that the patient is required to fill in those things.

Paul MahlerFor less tech savvy patients I keep getting calls or having them ask me in the waiting room what the password is and I have to tell them that they need to create it or they'll have worked on the intakes out of the office and then come in and ask me for the password and I have to tell them that it's the one they created when they started and they don't remember what they typed in at that time

CallieWe are having multiple New Patients not complete forms because they do not want to take the time to create a password , also they do not check their email as much as they do Text. We have received several intakes practically blank. I have to call the patient and ask them to complete the paperwork . We ended up having them fill out papers in the office. Also the Intake forms are not appropriate for Pediatric Patients.

Dr. HolmanThis requirement isn’t necessary on many EHRs and only adds unnecessary steps. Patients should be able to complete an intake and schedule directly from our website without needing a separate link. The goal is to make this process as simple and seamless as possible.
Ideal workflow:
1. The patient visits a page on our website with a ChiroFusion widget, similar to other companies.
2. They enter their basic information and schedule a new patient appointment.
3. They complete the intake form immediately afterward.
Summary: Patients stay on our website the entire time. They don’t have to search their email for a link, they can use any email across multiple CF clinics, and they don’t need to create or match a username and password.
Everything should happen with as few clicks and as little user friction as possible. The easier it is for patients to get started, the better their experience and the higher the conversion rate for the clinic.
