Report Request- Patient's who have not returned

We need a report that shows patients who have not returned in 3 months/6months etc or something to better track patients who cancel/no show and who do NOT reschedule. This would not pull inactive/deceased patients and the system would be be able to see who was on a plan that is no longer showing up.
