  Show when an appointment was created and by whom. Also show edit history (or at least most recent edit) in appointment information window.

  To be able to tell who scheduled, the doctor, the staff or the patient. Since some patients like to say, I did not make this appointment. Or if staff made, who to be able to talk to about how they are scheduling.
